The Hospice Hub is a grassroots organization connecting end of life care for our greater community – Perth, Smiths Falls, Westport, Portland and surrounding areas. Incorporated in March of 2023 we are in our early stages. We currently offer system navigation; bereavement groups as well as one-on-one bereavement support and advance care planning. Our first group of volunteers have completed their extensive training and will begin home visits once the
clinical coordination team is in place, offering support to people wherever they call home. In the next three years, we aspire to open a Hospice Residence to serve our communities.
Our services are provided to the client and their family at no charge. We are funded solely by donations and fundraising activities.
